Health Exams

Preventative measures such as immunizations help your child to avoid deadly diseases, but regular visits to your child's healthcare provider, when your child is well are the best way to maintain your child's good health. Routine exams can help identify any problems in your child's health or growth and development.

All students up to the age of nine are required by state law K.A.R. 72-5211 to submit evidence that they have had a health assessment (HF-039) done prior to entering kindergarten if attending a Kansas school or within the last 12 months of enrolling in the district for the first time. This assessment must be completed by a nurse who is KBH certified or by a physician or a person acting under the direction of a physician.

This form is completed during the online enrollment process. This form is required to be filled out annually to keep our records updated. 

Physical for Non-Athletic Activities

Athletic Physicals

The Kansas State High School Activities Association requires an annual physical examination to be eligible for participation in the middle or high school interscholastic athletic program. Physical forms are available in all middle and high school offices. Signed forms must be on file in the school office prior to participation in practice or tryouts and must be dated after May 1, preceding the school year for which it is applicable.
